After you register online you will be prompted to fill out intake paperwork
(General Information, WHODAS, Biospychosocial).

Please note that scheduling online does not automatically make you a client of Life Solutions Counseling. We will get back to you within 24 hrs to confirm your appointment, gather additional information as needed, or refer you to another provider if we can not help you. You can avoid this process by calling us first for a 15-20 minute free phone consultation.
